Arizona Exemption Certificate Form 5000
This is the Arizona State exemption certificate form, broken out by state. Arizona State exemptions may change every month, and this exemption is only current for the month in which it was downloaded.
This exemption Certificate is prescribed by the Department of Revenue pursuant to ARS ยง 42-5009. The purpose of the certificate is to document tax-exempt transactions with qualified purchasers. It is to be filled out completely by the purchaser and furnished to the vendor. The vendor shall retain this Certificate for single transactions or for specified periods as indicated below. Incomplete Certificates are not considered to be accepted in good faith. Only one form of exemption can be claimed on a certificate.
Arizona Exemption Certificate Form 5006

This Certificate is only for use by purchasers claiming full or partial exemption for overhead materials for use by a government contractor who is a manufacturer, modifier, assembler or repairer, or a direct subcontractor of the government contractor, pursuant to a contract between the government contractor and the United States government in which overhead is covered by the appropriate title passing clauses.
