Mail Your 1095 Forms by March 31, 2016

 New reporting requirements for the Affordable Care Act now in effect.

The Affordable Care Act increases both health care coverage and paperwork. The Marketplace must provide a 1095 form to the individuals it covers. Additionally, health insurance providers, other coverage providers, and certain businesses are required to provide 1095 forms to covered individuals or employees. The forms will help the IRS determine who has health insurance and who does not. Under the Affordable Care Act, many people without coverage (and their employers) must pay a penalty.

There are three 1095 forms:

  • Form 1095-A, Health Insurance Marketplace Statement. The Marketplace must send this form to individuals receiving health care through the Marketplace; the deadline for the Marketplace to provide the form is February 1, 2016. This form is for Marketplace enrollees.
  • Form 1095-B, Health Coverage. This form verifies health care coverage. Health insurance providers and other coverage providers (such as businesses with fewer than 50 employees) must send this form to the individuals they cover, along with information about who was covered and dates of coverage, by March 31, 2016. This form is not for Marketplace enrollees.
  •  Form 1095-C, Employer-Provided Health Insurance Offer and Coverage. Certain employers (Applicable Large Employee Members) that offer “self-insured coverage” send this form to certain employees and to the IRS. It contains information about the coverage offered to the employee. Employers must provide these forms to employees by March 31, 2016. This form is not for Marketplace enrollees.

Forms 1095-B, 1094-B, 1095-C and 1094-C must be filed with the IRS by May 31, 2016. For electronic filers, the due date is June 30, 2016. Additional information.
